Nurse/HCA Appointment Request

Do not use this form to request any urgent appointments, please contact the surgery on 01938 553118. This form can be used to request any of the stated routine HCA/Practice Nurse appointments, and we aim to respond by the end of the following working day. 

You can request:

• Smear test (following receipt of letter from Cervical Screening Wales)
• Zoladex, Depo-provera and b12 injections that you have on a regular basis (please ensure required date is stated upon requesting)
• Dressing change/removal of stitches (please ensure required date is stated)
• BP check (that has been requested by a clinician)
• Blood tests (that are required on a regular basis such as for medications, have been requested by a clinician at the surgery, or you have been provided blood forms from a hospital/clinic to have the test completed with your GP - if they have been requested for a specific date, pelase ensure this is stated when requesting the appointment).
• Shingles and Pneumonia vaccinations (following receipt of an invite letter from the surgery).
• Asthma reviews (these will be telephone calls initially unless otherwise stated by the surgery).
• Pre-assessment, 20 minute appointment with a Health Care Assistant (following receipt of an invite letter from the surgery)

Due to availability, we will not always be able to book an appointment on the exact date or time requested. We will aim to book an appointment that most macthes your request, but you can always contact us to arrange an alternative if required.

You can also book a variety of appointments online via My Health Online, without the need to wait for response from the surgery on avialable appointments. New users are required to complete a registration form to gain full access to the service, available from our Reception, or can be downloaded online here. Once complete these forms must be returned to our Reception Team, and ID verification provided.
