Do not use this form to request any urgent appointments, please contact the surgery on 01938 553118. This form can be used to request any of the stated routine HCA/Practice Nurse appointments, and we aim to respond by the end of the following working day. 

Booking links will be sent via SMS / email following your request.

You can also book a variety of appointments online via the NHS Wales App without the need to wait for response from the surgery on available appointments. New users are required to complete an identity verification via the app to gain full access to the service.
